Responsibilities

• Assess patients on admission and throughout care; document conditions and notify physicians as needed.
• Plan, direct, and document care, education, and discharge needs in collaboration with the care team.
• Prepare patients for procedures; report changes to supervisors and physicians; follow safety protocols.
• Reassess patients after interventions and modify care as needed to meet evolving needs.
• Provide clear bedside handoffs and ensure continuity of care during shift changes.
