The Registered Nurse (RN) provides comprehensive nursing and case management services to individuals with behavioral health and complex medical needs. The RN works collaboratively with clients, families, physicians, ARNPs, primary care providers, and other members of the treatment team to coordinate high-quality, individualized care.
The primary goal of this position is to promote each client's health, stability, independence, and overall functioning by coordinating psychiatric, medical, medication, and supportive services in an efficient and effective manner.
The RN is responsible for monitoring clients' health status and progress, coordinating and overseeing medications, identifying medical concerns and risk factors, providing education and support, and ensuring appropriate follow-up with healthcare providers. This position will travel between Madison, Jefferson, Taylor, Wakulla, Liberty, and Franklin counties.
Conduct comprehensive nursing assessments that consider the relationship between physical and mental health. Identify medical concerns, risk factors, barriers to care, and other healthcare needs. Coordinate appropriate services and follow-up care with internal and external providers. Complete required assessments and documentation in accordance with agency and Client Record Handbook guidelines.
Participate in the development and implementation of individualized service plans in partnership with clients, youth, and families. Establish measurable goals and objectives that address identified behavioral health, medical, and family needs. Review and update service plans as required and whenever significant changes occur in the client's condition or circumstances.
Implement prescribed medical and psychopharmacological treatment in accordance with agency policies and procedures. Responsibilities may include transcribing, administering, monitoring, evaluating, and documenting prescribed medications. Monitor clients for medication effectiveness, adverse reactions, side effects, and other medical concerns. Assist with the management of medications, pharmaceuticals, and medical supplies.
Provide direct nursing care, including patient assessments, medication administration, care planning, crisis intervention, health education, prevention, and wellness support. Serve as a client advocate and liaison and make appropriate referrals and consultations within and outside the agency. Provide education and support to clients and families regarding symptom management, medications, healthcare needs, and strategies that promote greater independence and stability.
Maintain accurate, timely, and thorough documentation of all services provided. Complete and finalize documentation in Avatar within required timeframes and in accordance with Client Record Handbook guidelines. Progress notes should clearly reflect services provided, their relationship to the client's treatment goals, and the client's progress toward those goals.
Work collaboratively with the multidisciplinary treatment team and other healthcare providers to promote coordinated, comprehensive, and person-centered care. Participate in team meetings, quality improvement activities, and other initiatives designed to enhance client outcomes and service delivery.
