St. Gabriel's Community in Minnesota is seeking a licensed Physical Therapist to evaluate residents, develop and revise care plans, and implement therapeutic interventions. You will plan discharges and anticipate long-term and short-term needs while educating families and caregivers on the care plan.
Qualifications include current state licensure in Physical Therapy and a Bachelor’s degree. This is a full-time, on-site role with a comprehensive benefits package.
The Physical Therapist is responsible and accountable for residents’ evaluations, ongoing assessments, and the planning and implementation of therapy interventions.
Develops and revises plan of care based on the residents’ needs and establishes functional goals, outcomes, and discharge plans. Plans for residents’ discharge and anticipates for long term and/or short term residents’ needs. Educates other health care team members, residents, family, and caregivers on information regarding the care plan for the residents.
