St. Gabriel's Community in Minnesota is seeking a licensed Home Health Physical Therapist to evaluate residents, develop and implement individualized therapy plans, and coordinate care with an interdisciplinary team. The role involves education for residents and families, and ensuring documentation and billing align with current standards.
Qualifications include an active Physical Therapy license in good standing and a Bachelor's Degree. Prior long-term care experience is preferred.
The Home Health Physical Therapist is a clinically competent, licensed, professional of his/her discipline who assumes responsibility and accountability for a group of Home Care/Home Health residents/patients through the process of evaluation, ongoing assessment, planning, and implementation of therapy interventions. Effectively and professionally interacts with residents/patients, family members, responsible party and other health care team members, while maintaining standards of professional conduct. This position must comply with regulatory standards of the American Association of Physical Therapy, the Department of Health, and any other professional guidelines or standards.
