Benedictine Living in Minnesota is seeking a licensed Home Health Physical Therapist to evaluate and treat residents in a home care setting. You will perform initial assessments, develop plans of care, and deliver therapies aligned with physician orders and facility policies.
You must hold a current PT license in good standing and a Bachelor's degree. Experience in long-term care is preferred. The role emphasizes patient education, documentation, and coordination with the interdisciplinary team.
The Home Health Physical Therapist is a clinically competent, licensed, professional of his/her discipline who assumes responsibility and accountability for a group of Home Care/Home Health residents/patients through the process of evaluation, ongoing assessment, planning, and implementation of therapy interventions. Effectively and professionally interacts with residents/patients, family members, responsible party and other health care team members, while maintaining standards of professional conduct. This position must comply with regulatory standards of the American Association of Physical Therapy, the Department of Health, and any other professional guidelines or standards.
