Job summary

Qualifications

• Must possess a current, valid RN license in state of practice; BLS certification required.
• Experience in the clinical area for which applying is desired; acute care setting experience within the past 12 months preferred.
• In a Home Health setting, operate a personal vehicle and maintain valid driver’s license if applicable.

Responsibilities

• Assess patient needs and discharge planning; review history, tests, and inform physician as needed.
• Formulate and adjust a plan of care in collaboration with the multidisciplinary team.
• Provide direct patient care and supervise staff as needed; ensure communication across caregivers.
