Job summary

Center for Human Development (CHD) is seeking an Outreach Caseworker to join the PILOTS team in Torrington, CT. The role focuses on helping participants live independently by addressing mental/physical health, substance use, employment and housing needs.

Duties include securing housing, budgeting, obtaining entitlements, crisis intervention and job-search support. Schedule is 1st shift, Mon-Fri, 8:30am-4:30pm.

Qualifications

• High school diploma or GED required
• Experience with psychiatric disorders or co-occurring population preferred
• Must be comfortable engaging clients in the community for majority of the position
• Field visits within the surrounding communities to see clients
• Registered & insured vehicle and approved driving record check
• Approved criminal background check

Responsibilities

Description

CHD is seeking an Outreach Caseworker to join our growing Planning and Implementing Housing Options for Long-term Success (PILOTS) team in Torrington, CT. Through this program, the Outreach Caseworker PILOTS staff will use a counseling approach that uses both motivational interviewing and harm-reduction.

YOUR ROLE AS A PILOTS SUPPORT SPECIALIST:

As a PILOTS Support Specialist, you will assist program participants to live independently in the community by addressing their mental and physical health, substance use, employment and housing needs. PILOT’s staff use a counseling approach that uses both motivational interviewing and harm-reduction. You will help the participants with:

SCHEDULE:

1st shift, 8:30am-4:30pm. Days on: Monday, Tuesday, Wednesday, Thursday, Friday. Days off: Saturday and Sunday.
