Job summary

Parkview Health in Fort Wayne, IN is seeking a Caseworker II to provide monitoring and supportive intervention across child, adolescent, and adult clients, coordinating with the treatment team and families.

You will conduct home visits, facilitate crisis response, and assist in developing and carrying out individualized treatment plans, while collaborating with schools and community resources.

Qualifications

• Bachelor's degree in social work, psychology, sociology, human services, or related field.
• At least one year of experience in a mental health environment.
• Valid state driver's license, reliable transportation, and proof of auto insurance.
• CPR required within 30 days of employment.
• Non-crisis intervention within 60 days of hire.
• CANS 0–5 within 30 days for Bluffton/Decatur offices; CANS 5–17 for other areas.

Responsibilities

• Provide monitoring and supportive intervention to child, adolescent, and adult clients.
• Communicate progress and problems to the treatment team and families.
• Arrange transportation services when needed.
• Respond to crisis calls and provide crisis intervention with the team.
• Participate in developing and carrying out treatment plans; conduct home visits and community liaisons; work with school systems.
