Qualifications

Responsabilités

• Make home visits to interview clients and support networks.
• Compile and evaluate information and prepare care plans.
• Monitor clients’ progress toward goals.
• Document all substantive client contacts in records.
• Ensure duties comply with federal, state, and county regulations.
• Collaborate with departments and community agencies to provide services.
• Review information to determine eligibility for programs.
• Attend meetings and trainings as needed.
• Consult with supervisor on critical decisions.
