Job summary

The Children’s Home is seeking a YES TAY Case Worker in Goshen, NY. You will work with clients, families, staff, and community partners to support family stability and safety.

Responsibilities include counseling, assessments, case planning, and collaboration with DSS and schools, while maintaining accurate records. A Bachelor's degree is required and Spanish bilingualism is preferred.

Qualifications

• Bachelor's Degree in human services or related field is required.
• Experience in human services or related field preferred.
• Driver's License and ability to maintain insurability throughout employment required.

• Providing counseling to families related to presenting needs and challenges.
• Completing assessments - including but not limited to the FASP, inputting progress notes into Connections.
• Providing face-to-face contact, transportation, case planning, and implementation of the case plan and ensure that other service providers do the same.
• Facilitating treatment planning conferences.
• Maintaining good relationships through collaboration and advocacy on client's behalf with DSS, community agencies, schools, and other service providers.
• Ensuring timely, accurate recording of all components of clients' records, including; progress notes, treatment planning conferences, case specific materials and Family Assessment and Service Plans.
• Maintaining a caseload of 1:16

Position Summary:

The YES TAY Case Worker is responsible for working effectively with clients, families, staff, and community contacts with a diversity of backgrounds and life situations. This role will provide support to families related to presenting needs and challenges identified through the treatment planning process. The goal of the YES TAY Case Worker is to keep families together, while continually assessing for risk and safety issues. As a YES TAY Caseworker, you will have the opportunity to help strengthen families, stabilize and maintain family functioning.
