Job summary

The Children\'s Home in New York is seeking a preventive service case worker to support families by assessing risks, strengthening family functioning, and fostering healthy parent-child relationships. Under the supervision of the Program Supervisor, you will be the primary service provider aligned with the agency\'s family-centered approach.

You will collaborate with DSS and schools, provide parenting skills groups and counseling, complete assessments and documentation, and participate in

• Bachelor's Degree in human services or related field required.
• Some experience working with children or families preferred.
• Driver's License and ability to maintain insurability throughout employment required.

Responsibilities

• Maintaining good relationships through collaboration and advocacy on client's behalf with DSS, community agencies, schools, and other service providers.
• Provide parenting skills groups, advocacy, and counseling for families, addressing needs
• Completing assessments - including but not limited the FASP, inputting progress notes
• Maintain comprehensive and timely documentation, including assessments (e.g., FASP), progress notes, and case-specific materials, ensuring accuracy in client records.
• Facilitate and participate in regular planning conferences, case planning, and service implementation, ensuring the involvement of all relevant service providers.
• Manage a caseload of 1:16

They assess risk and safety, work to strengthen and stabilize family functioning, and promote healthy parent-child relationships. Guided by the Program Supervisor, the case worker develops therapeutic relationships and serves as the primary service provider, aligning with the agency's family-centered approach.
