GENERAL DESCRIPTION

Performs moderately complex (journey-level) casework. Work involves serving as a liaison between clients and their families in obtaining assistance and rehabilitative treatment. Works under general supervision with limited latitude for the use of initiative and independent judgment.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Interviews clients, their families, law enforcement officials, schools, and health and social welfare institutions to obtain client background information.
  • Identifies the psychosocial, economic, and physical needs of clients.
  • Assesses clients’ support systems and available community resources, schools, and facilities.
  • Provides casework services during client admission, discharge, or furlough.
  • Counsels clients and their families and coordinates placements of clients with community facilities.
  • Serves as liaison between clients and their families, state and judicial systems, and facility or treatment providers.
  • Assists with developing and updating individual treatment plans, and monitors the observance of clients’ rights.
  • Meets and maintains all minimum service requirements according to Center requirements, Resilience Disease Management Design, and Texas Administrative Codes.
  • Documents case records, assessments, person-centered recovery plans, and progress notes according to Center requirements and Texas Administrative Codes; maintains complete and accurate consumer medical records and keeps all such information confidential.
  • May participate in individual, group, or family therapy sessions.
  • Ensures compliance with all required training.
  • May participate as an active member of an interdisciplinary team.
  • Maintains work behavior compatible with Center value statement and Policies and Procedures Manual.
  • Demonstrates high ethical standards, trustworthiness, and personal integrity.
  • Maintains regular attendance, dependability, and promptness for the 8-5 work day or agreed upon hours 100% of the time.
  • Completes professional development training and continuing education as required by professional standards and the Center.
  • Frequent daily driving within the catchment area is required.
GENERAL QUALIFICATIONS
Experience/Education/Licensing/Certification
  • Experience in a field relating to the agency program.
  • Graduation from an accredited four-year college or university with major coursework in a relevant field.
  • Experience and education may not be substituted for one another.
Knowledge/Skills/Abilities
  • Knowledge of mental illness, emotional components of physical illness, rehabilitation methods, casework principles, community resources, and trauma-informed practices.
  • Skill in interviewing.
  • Proficiency in computer programs such as Microsoft Word and Excel.
Relationship Skills
  • Effective verbal and written communication.
  • Works cooperatively and productively with supervisors, coworkers, and other agencies, contributing to teamwork and trust.
  • Plans and schedules work and implements directives without constant supervision.
  • Maintains a professional appearance and conduct when representing the Center.
  • Establishes and maintains effective working relationships with staff, agencies, and the public using tact, courtesy, and good judgment.
Additional Requirements
  • Must have and maintain a valid driver’s license with an acceptable driving record.
  • Regular attendance and reliability are critical to business operations.
WORK ENVIRONMENT AND FUNCTIONAL REQUIREMENTS

Position requires the ability to sit, stand, and walk for extended periods. Perform repetitive tasks such as typing, filing, and data entry. Lift and/or move up to 15 pounds. Interact frequently with consumers, staff, and the public in a professional manner. Respond to challenging situations and de-escalate interactions in accordance with training and Central Counties Services policies. Maintain sufficient mental and emotional health to manage stress and meet inherent job stressors. Work in an environment with frequent interruptions and changing priorities. Perform essential functions with or without reasonable accommodation in accordance with the Americans with Disabilities Act.
