Walnut Street Center in Massachusetts is seeking a Case Worker to support adults with developmental disabilities who live independently or with their families. You will help individuals achieve independence and coordinate services to meet their needs.

The role involves balancing caseloads, training individuals and staff, coordinating medical and benefits, and developing treatment plans. It also includes financial guidance and transportation support to empower personal growth.

Qualifications

• Associate's degree preferred.
• Valid driver’s license; willing to travel up to 100% of the time.
• Ability to pass background checks.
• Ability to communicate effectively in English.
• Ability to independently complete computer-based training.

Responsabilités

• Balance a regular caseload dependent on hours of support.
• Provide skill training to individuals and outside staff; coordinate medical and benefits.
• Promote activities leading to personal growth and accomplishment.
• Develop therapeutic relationships with individuals and document treatment plans.
• Assist with development and evaluation of Individual Treatment Plans (ISP).
• Support and monitor individuals' funds and transportation needs.
