Job summary

Actalent is hiring a Case Manager I in Hazelwood, MO to work with a multidisciplinary healthcare team. You will perform bio-psychosocial assessments, develop individualized care plans, arrange referrals, and monitor progress with a focus on HIV/AIDS education and adherence.

The role includes data tasks and regular team collaboration. The position is contract-to-hire, onsite in Hazelwood with a day shift and potential remote day once per week.

Qualifications

• Bachelor’s degree in a health-related field such as social work, counseling, sociology, or psychology.
• Experience in case management, managed care, and care management.
• Strong problem-solving and organizational skills.
• Ability to work well with clients and maintain a customer service attitude.
• Creativity and the ability to work independently without much guidance.
• Experience with charting and paperwork management.

Job Title: Case Manager I

The Case Manager I will work collaboratively with a multidisciplinary healthcare team to provide case management services to clients. This role involves conducting comprehensive bio-psychosocial culturally-sensitive assessments, synthesizing program goals, and creating individualized plans of care.

Work Environment

The position is based in an office setting with a day shift from 8:30 am to 4:30 pm or 9 am to 5 pm, with one remote day per week. The team is collaborative, working well together, and an open-door policy is encouraged. The training provided is robust, and there are opportunities for advancement within the organization after nine months in the role.

Job Type & Location

This is a Contract to Hire position based out of Hazelwood, MO.

Pay And Benefits

The pay range for this position is $21.00 - $25.00/hr. Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
