Coalition for Hispanic Family Services in Queens is seeking a Queens School-Based Early Support Case Planner to provide case management and counseling to children and families in our partner schools in Queens School District 30.
The role requires a Bachelor's degree in Social Work or related field, with 1+ year of experience in school settings or child welfare; strong communication and collaboration with school staff are essential. Spanish-speaking is preferred for serving the community.
CHFS is a community- based, family-focused organization of color providing an array of services to children and families in North Brooklyn and Western Queens. For over 34 years, CHFS has worked with vulnerable populations that include children in foster care, seriously emotionally disturbed children, youth, and families at risk of abuse and neglect. Our strength-based model also supports progressive youth development programs, family support services and community school partnerships, the provision of accessible, high-quality family focused services to families of color is central to our holistic approach of strengthening families in a culturally competent manner. Our family- centered work serves to strengthen communities one family at a time.
Funded by the NYC Administration for Children Service (ACS), the goal of the School-Based Family Support program is to develop strong partnerships between schools, communities, and parents/caregivers in order to help children thrive, keep them safe in their homes and communities, as well as empower parents to become active participants in their children’s education.
A candidate must have a Bachelor degree in Social Work or related Human Service field, with one or more years prior experience in school settings, child welfare or other social services, including day care, youth services or other related programs that involves direct work and contact with children and families. Strong verbal and written communication skills required. Must be computer literate. Familiarity with Connections and Promis Systems are a plus.
Spanish-speaking strongly preferred, as population served is mostly Spanish-speaking.
