Job summary

Coalition for Hispanic Family Services in Queens is seeking a Queens School-Based Early Support Case Planner to provide case management and counseling to children and families in our partner schools in Queens School District 30.

The role requires a Bachelor's degree in Social Work or related field, with 1+ year of experience in school settings or child welfare; strong communication and collaboration with school staff are essential. Spanish-speaking is preferred for serving the community.

Qualifications

• Bachelor's degree in Social Work or related Human Service field is required.
• One or more years of prior experience in school settings, child welfare or other social services.
• Strong verbal and written communication skills are required.
• Must be computer literate; familiarity with Connections and Promis Systems is a plus.

Responsibilities

• Maintain a caseload of up to 12 families.
• Conduct in-person contacts in the family home, school and/or program site.
• Assess children’s well-being and needs in educational, behavioral, and medical areas.
• Collaborate with school staff to support children’s academic needs.
• Provide case management support and counseling to families served by partner schools.
• Attend monthly case conferences and weekly supervision for progress review.

Work Hours: 35 Hours per week

ABOUT THE SCHOOL-BASED FAMILY SUPPORT PROGRAM:

Funded by the NYC Administration for Children Service (ACS), the goal of the School-Based Family Support program is to develop strong partnerships between schools, communities, and parents/caregivers in order to help children thrive, keep them safe in their homes and communities, as well as empower parents to become active participants in their children’s education.

POSITION SUMMARY:

1. The School-Based Family Support Case Planner’s general responsibilities focuses on providing broad case management support and counseling to children and families associated with our partner schools in Queens School District 30.

RESPONSIBILITIES:

• Maintain a caseload of up to 12 families
• Conduct in-person contacts in the family home, school and/or program site
• Assess children’s general well-being around educational, behavioral, and medical needs
• Assess, refer, advocate and support children and families’ participation with community-based resources
• Assess children/family’s needs for to supportive and concrete services, housing, financial assistance, medical, immigration, legal, etc.
• Maintain ongoing, collaborative partnerships with school staff to support children’s academic needs
• Partner with Case Aide in aiding and supporting families in obtaining resources, support services, and advocacy assistance
• Complete all casework documentations timely and meet program standards
• Participate in monthly case conferences to discuss family case progress, and/or develop intervention strategies to improve family outcomes
• Participate in weekly Supervision for support, coaching and discuss client-family progress
• Implement Quality Assurance data to drive case practice and improve performance and achieve outcomes
• Participate in internal and external continuous quality improvement processes
• Attend ACS and agency in-house trainings as required
• Complete special projects as assigned by the Unit Supervisor, and/or Program Director
• One to two late nights as may be required
• Flexible hours, including Saturdays as may be required

MINIMUM REQUIREMENTS:

A candidate must have a Bachelor degree in Social Work or related Human Service field, with one or more years prior experience in school settings, child welfare or other social services, including day care, youth services or other related programs that involves direct work and contact with children and families. Strong verbal and written communication skills required. Must be computer literate. Familiarity with Connections and Promis Systems are a plus.

PREFERRED BUT NOT REQUIRED:

Spanish-speaking strongly preferred, as population served is mostly Spanish-speaking.
