Job summary

Puerto Rican Family Institute in New York is seeking a Family Case Planner to strengthen families, assess and address children's needs, and guide caretakers under supervision.

Qualifications

  • BA/BS/BSW degree preferred.

  • Must maintain confidentiality and have the ability to exercise a high level of judgment/discretion.
  • Ability to create and maintain well‑organized administrative and operational systems.
  • Knowledge and proficiency of MS Word, Excel, PowerPoint, and willingness to learn new programs as needed.
  • Must be able to work independently and manage multiple tasks in a fast‑paced environment.
  • Ability to work collaboratively with a variety of people, as well as an aptitude for teamwork.
  • Ability to use public transportation for travel to program locations throughout New York City.
  • Spanish speaking a plus.

Responsibilities

  • Conduct initial assessment of family needs and develop a preliminary service plan with supervision.
  • Engage in Community Outreach Advocacy.
  • Provide comprehensive case planning, family counseling, and other casework modalities under supervision.
  • Coordinate delivery of subordinate services with the Family Aide.
  • Monitor linkages with Community Services Providers and school visits.
  • Ensure the safety of all children served and follow clinician recommendations.
  • Maintain timely progress notes and FASPS; complete weekly submissions.
  • Conduct Parenting Skills Training and other groups.
  • Perform basic administrative tasks related to caseload and records.
  • Participate in weekly supervisory conferences and groups.
  • Other related duties as assigned.

Job Location: Bronx Family Support Program - Bronx, NY 10456

Salary Range: $50,000.00 - $57,000.00 (Salary/year)

Position Summary

Family Case Planners help strengthen families, stabilize and maintain family functioning by assisting caretakers to meet the children's needs, prevent abuse/neglect, enhance parenting, coping, communication, and insight skills with the guidance of the Program’s Supervisor and Director.

Major Duties
  • Responsible for initial assessment of family needs pertaining placement risk factors in children, problems, needs, capacity to use preventive services, and the development of a preliminary service plan in close coordination with Supervisor.
  • Responsible for Community Outreach Advocacy.
  • Responsible for the provision of adequate case planning, family counseling, and other casework modalities under close coordination of supervision.
  • Responsible for the planning, coordinator, and delivery of subordinate services to the family through the help of the Family Aide when available.
  • Responsible for monitoring linkages with the Community Services Providers, visits to schools and other providers.
  • Responsible for securing the safety of all children served.
  • Facilitate client’s use of the psychiatric and psychological services, and the follow-up of the clinician’s recommendations.
  • Responsible for at least three face‑to‑face contacts monthly; two office visits and one in the home; making adequate outreach for resistant and/or reluctant clients. All school neglect cases are to have two contacts yearly with school officials. 2nd home visit is required if there is a child under 6 months in the home.
  • Responsible for monitoring and close supervision of case management of all preventive services.
  • Completion of all FASPS on time, including weekly submission of progress notes.
  • To perform the basic administrative tasks related to caseload, such as statistics, recordings, FASPS, progress notes and contact logs.
  • Participate actively in weekly individual supervisory conference and group supervision.
  • Conduct groups (Parenting Skills Training; Adolescent and Children’s Group).
  • Other tasks as needed.
