Job Title:Case Planner

Program:Family Connections

Salary: $53,000- $55,000 (based on experience and educational achievements)

General Responsibilities:

CGS is looking for an Creative and Empathetic Case Planner for our Prevention Program. As a Case Planner you are responsible for identifying appropriate treatment or service providers and facilitates client referrals to internal and external and/ or community linkage programs. Evaluates clients to assess risks and develop case plans.

Required Skills:

• BA/BSW Degree
• Child Welfare Experience

Desired Skills:

• Some experience with specific client population or specialty (domestic violence, substance abuse, community outreach, etc.)
• Case management
• Bi-lingual Spanish a plus!
