Job summary

University Settlement Society of New York is seeking a full-time Case Planner for the Prevention Program in Brooklyn. You will maintain weekly contact with a caseload of families, perform safety assessments, and document progress with Mobility Mentoring in CONNECTIONS & PROMIS.

Qualifications include a Bachelor's degree in Social Work or related field and at least two years of experience with children/families.

Qualifications

Bachelor's degree in Social Work or other human service field is required.
Two years of related experience working with children and/or families preferred in child welfare, juvenile justice, or mental health settings.
Excellent organizational skills and ability to meet deadlines.
Ability to work flexible hours including evenings and some weekends.
Strong communication and documentation skills; commitment to diversity, equity, and inclusion.

Responsibilities

Maintain weekly contact with a caseload of families in home, community, or office settings.
Complete safety and risk assessments in accordance with program mandates.
Integrate Mobility Mentoring into work with families and document efficacy.
Attend required trainings and apply learning in family work.
Provide casework counseling within a trauma-informed framework and monitor progress toward goals.
Engage families in case planning and coordinate with ACS and other partners.
Document all information and ensure timely data entry into CONNECTIONS & PROMIS.
