Job summary

Basecamp Recovery Center in Columbus, Ohio seeks a CDCA Case Manager to provide recovery-oriented case management, develop individualized service plans, and connect patients with housing, employment, and community resources across Franklinton and Whitehall sites.

Qualifications

  • High School Diploma or GED required; related degrees preferred.
  • Active CDCA credential or QBHS certification required and maintained.
  • 1+ year of case management or behavioral health experience preferred.
  • Experience with housing instability or complex social needs is a plus.

Responsibilities

  • Develop and implement individualized case management plans with assessments and service planning.
  • Maintain an active caseload at Franklinton and Whitehall, coordinating services.
  • Link patients to housing, employment, childcare, education, and other social determinants of health.
  • Refer to mental health, medical, legal, and social services as needed.
  • Attend meetings, advocate for patients, and coordinate care with clinical teams.
  • Deliver substance use and mental health interventions, applying trauma-informed approaches.
  • Maintain accurate, confidential records and comply with HIPAA, 42 CFR Part 2, and OCDPB.
  • Participate in supervision, quality improvement, and caseload management.

This role is responsible for developing and implementing individualized case management plans, connecting patients to community resources, and serving as an advocate to ensure each patient has access to the services and supports they need to thrive.The CDCA Case Manager works as part of BRC's clinical and case management team under the direction of the Case Manager Lead, upholding BRC's core values of Love, Community, Consistency, Honesty, and Hope in every patient interaction. This role requires an active Chemical Dependency Counselor Assistant (CDCA) credential and operates in accordance with Ohio Chemical Dependency Professionals Board (OCDPB) standards, OhioMHAS guidelines, and CARF accreditation requirements.ESSENTIAL JOB FUNCTIONSCase Management & Service PlanningDevelop and implement individualized case management plans in collaboration with the clinical team, including comprehensive assessments, goal setting, service planning, progress monitoring, and support for patient independence.Maintain an active patient caseload at assigned BRC sites, ensuring each patient receives timely, consistent, and coordinated support throughout their engagement with BRC services.Provide comprehensive linkage to community resources to address patients' needs including housing, employment, childcare, education, clothing, food, transportation, financial stability, and other social determinants of health.Serve as a liaison between patients and external agencies, making referrals to mental health, legal, medical, and social service resources as needed to ensure access to appropriate programming and support.Attend patient meetings, court appearances, school meetings, and other appointments as needed to support patient advocacy and service coordination.Support colleagues by assisting with caseloads during absences and collaborating on group case management as directed by the Case Manager Lead.Clinical Support & InterventionDeliver high-quality substance use, mental health, and supportive intervention services to patients in alignment with BRC's evidence-based treatment model and CDCA scope of practice.Provide prevention, consultation, and educational services to patients and their support systems as directed.Apply trauma-informed, recovery-oriented principles in all patient interactions, recognizing the unique strengths and challenges of each individual served.Collaborate actively with BRC's clinical team — including counselors, nursing staff, and peer support — to ensure integrated, whole-person care for every patient.Stay informed of emerging treatment strategies, best practices, and developments in substance use disorder and behavioral health through ongoing professional development and continuing education required for CDCA renewal.Documentation & ComplianceMaintain accurate, timely, and confidential case records and documentation, including progress notes, service plans, and collateral correspondence, in accordance with BRC policy, OhioMHAS standards, and CARF accreditation requirements.Comply with all applicable confidentiality standards including 42 CFR Part 2 and HIPAA in all patient interactions and documentation.Adhere to all applicable state and local laws, agency policies and procedures, patient rights standards, safety protocols, and the OCDPB Code of Ethics governing CDCA practice.Contribute to BRC's Quality Improvement activities, including peer reviews, OhioMHAS compliance monitoring, and agency quality assurance processes.Maintain active CDCA credential in good standing with the Ohio Chemical Dependency Professionals Board (OCDPB), including all required continuing education hours, throughout employment.Teamwork & Professional ConductActively participate in supervision sessions, case management meetings, treatment team meetings, and other agency-required meetings as scheduled.Uphold BRC's mission and core values — Love, Community, Consistency, Honesty, and Hope — in all interactions with patients, staff, and community partners.Maintain professional relationships with patients, staff, and external partners; model appropriate boundaries and ethical conduct at all times.Communicate proactively with the Case Manager Lead regarding caseload concerns, patient safety issues, or operational challenges requiring escalation.Perform other related duties as assigned.Productivity RequirementMeet and maintain 60% productivity/impact monthly.QUALIFICATIONSEducationHigh School Diploma or GED required.Associate's or Bachelor's degree in Counseling, Social Work, Human Services, or a related field preferred; equivalent combination of education and experience considered.Required CredentialActive Chemical Dependency Counselor Assistant (CDCA) credential issued by the Ohio Chemical Dependency Professionals Board (OCDPB) OR a Qualified Behavioral Health Specialist (QBHS) certification is required.One of these credentials must be maintained in good standing throughout employment; lapse in credential status must be reported to the Case Manager Lead and HR immediately.ExperienceMinimum of one (1) year of experience in case management, social services, behavioral health, or a related field preferred.Experience working with individuals in recovery from substance use disorder and/or mental health conditions strongly preferred.Experience working with individuals experiencing housing instability, homelessness, or other complex social needs is a plus.Broad knowledge of Franklin County community and social service systems preferred.Knowledge, Skills & AbilitiesStrong interpersonal and communication skills; ability to engage effectively with individuals from diverse socioeconomic and cultural backgrounds.Effective oral and written communication skills including the ability to produce clear, accurate clinical documentation.Strong decision-making and problem-solving abilities; able to assess complex situations and respond appropriately within CDCA scope of practice.Conflict resolution skills and ability to de-escalate challenging situations with professionalism and empathy.Proficiency in Microsoft Office and electronic case management or health record systems; experience with BestNotes preferred.Ability to manage an active caseload, prioritize competing demands, and meet documentation deadlines in a fast-paced behavioral health environment.Knowledge of trauma-informed care principles, motivational interviewing, and recovery-oriented service delivery.Knowledge of OhioMHAS standards, CARF accreditation requirements, 42 CFR Part 2, and HIPAA as applicable to the CDCA case management role.WORK ENVIRONMENT & PHYSICAL REQUIREMENTSWork is performed at BRC's Franklinton and Whitehall sites; travel between locations and to community appointments may be required.Standard office and clinical environment with regular in-person patient contact.Sedentary to light physical activity; extended periods of computer use, phone communication, and in-person patient interaction.Must be able to work in an environment serving individuals in active recovery and/or treatment for substance use disorder;
