Job Duties and Responsibilities
  • Utizlizes a trauma-informed approach to client care.
  • Conducts initial and ongoing needs assessments and assist clients in identifying recovery goals.
  • Coordinates and links clients to appropriate treatment services, primary care, housing, employment, legal, social and recovery supports.
  • Monitor client progress towards treatment plan goals.
  • Utilizes motivational enhancement strategies to assist clients in achieving personal recovery goals.
  • Advocate on behalf of clients and support engagement in recovery-oriented and person-centered services.
  • Ensure accurate and timely documentation of all services in compliance with agency policy and regulatory standards.
  • Participates in case conferences, team meetings and trainings as required.
  • Aids in the development of individualized service plans in collaboration with the client and the interdisciplinary team.
  • Maintains strong clinical and ethical boundaries with clients.
  • Facilitates group counseling as required for training and development.
Qualifications
  • High school diploma or equivalent.
  • Associates or Bachelor’s degree in Psychology, Counseling, or a related Human Service Field preferred.
  • CDC-A certification required within 90 days of hire.
  • Valid driver's license and insurable driving record. Position requires community based work.
  • Must be willing to transport clients.
  • Minimum of one year of experience working with individuals with mental health or substance use disorders.
  • Strong interpersonal and communication skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Experience with writing clinical documentation.
  • Familiarity with trauma-informed care, harm reduction and motivational interviewing techniques.
  • Strong knowledge of community resources and systems.
  • Proficient with Microsoft 365 applications, including Office Suite, Outlook and Teams.
  • Adept at learning and navigating electronic health record systems.
