Job summary

The Doe Fund is hiring a Case Manager to support residents in a 151-unit permanent supportive housing program serving homeless individuals and families in the NYC area. You'll provide counseling, assessment, service planning and linkage to healthcare, benefits and community services.

Ideal candidates have a social work background, knowledge of HIV/AIDS and related services, and strong written and verbal communication skills to work with diverse residents and partners.

Qualifications

  • Bachelor's Degree in Social Work or relevant field.
  • Minimum of three years of post-Master's experience.
  • Working knowledge of HIV/AIDS, substance abuse, mental illness and homelessness.
  • Excellent case management skills with knowledge of NYC Metro benefits.
  • Strong ethics, confidentiality and integrity.
  • Excellent oral and written communication.
  • Proficient with email, Word, Excel and PowerPoint.

Responsibilities

  • Maintain a caseload and provide supportive case management to residents.
  • Maintain resident records in compliance with agency standards.
  • Conduct and review assessments and develop Service Plans.
  • Link residents to healthcare providers and monitor progress.
  • Provide counseling and crisis intervention as needed.
  • Assist residents in securing and maintaining benefits.
  • Coordinate with hospitals, substance abuse programs, HRA and mental health services.
  • Facilitate groups and activities promoting independent living skills.
  • Assist with rent payments and budgeting; document outcomes.

Our flagship workforce development program, Ready, Willing & Able (RWA), operates across 660 transitional housing beds and combines housing and comprehensive supportive services with paid work training, career preparation, and occupational skills training. The Doe Fund's innovative social enterprise model creates additional opportunities for New Yorkers to gain paid, hands-on experience while delivering essential services across the city, including supplemental sanitation, graffiti removal, and rain garden maintenance. We are also one of New York City's largest nonprofit developers of permanent affordable and supportive housing, with more than 2,100 apartments in operation or development, creating safe, dignified homes with supportive services and spaces that foster community and connection. The Doe Fund has an annual operating budget of $88 million and a full-time staff of over 500, a significant portion of whom are graduates of our programs. We are seeking a Case Manager for

The Doe Fund's newest permanent supportive housing program for homeless individuals and families living with HIV/AIDS. The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. This 151-unit residence will provide housing for 31 individuals referred by the New York City HIV/AIDS Services Administration (HASA), and the remaining 120 units will be apartments for moderate-income households.

The Case Manager is responsible for providing supportive case management services to the residents referred by HASA. The successful candidate will have strong clinical and communication skills along with experience working with the HIV/AIDS population.

RESPONSIBILITIES:
  • Maintain a caseload and provide supportive case management services to these individuals
  • Maintain resident records in compliance with agency and funding standards
  • Conduct and review assessments
  • Develop and implement individual Service Plans and assist residents with achieving their goals geared towards self-sufficiency
  • Monitor and ensure residents are linked to healthcare providers
  • Provide counseling and crisis intervention as necessary
  • Assist residents in securing and maintaining benefits
  • Maintain a liaison with community services including hospitals, substance abuse treatment programs, HRA, and mental health programs
  • Facilitate groups and activities that promote health and independent living skills development
  • Monitor and assist residents with rent payments and budgeting
  • Educate residents about approaches to mental health
  • Attend ongoing trainings to enhance the services provided to the residents
  • Assist with completion of reports as needed
  • Other duties as assigned
QUALIFICATIOns:
  • Bachelor's Degree in Social Work or relevant field
  • Minimum of three years of post-Master's experience
  • Ideal candidate must have working knowledge of HIV/AIDS, substance abuse, mental illness, and homelessness
  • Excellent case management skills, including knowledge of benefits in the NYC Metro area
  • Sensitivity to community issues and needs
  • Outstanding interpersonal and communication skills, both oral and written
  • Strong ethical standards, confidentiality, and personal integrity
  • Computer proficiency (i.e., email, Microsoft Word, Excel and PowerPoint)
