The Medical College of Wisconsin, Inc. seeks a Trauma Recovery Center Case Manager to provide trauma-informed case management for survivors of violent crime.

You will assess needs, develop individualized plans, coordinate care across medical, mental health, legal, and social services, and support engagement in care with a multidisciplinary team. Responsibilities include linking clients to housing, benefits, and legal resources, assisting with applications, and advocating within systems.

Qualifications

• Minimum Education: Associate’s degree; experience may substitute education.
• Minimum Experience: 2 years in case management or related social services.
• Bilingual/bicultural (e.g., Spanish/English) preferred.

Responsibilities

• Conduct assessments to identify client needs across multiple domains.
• Collaborate with clients and clinical staff to develop individualized service plans.
• Monitor and update client needs and goals throughout services.
• Provide ongoing case management to support stability and recovery.
• Link clients to shelter, housing, healthcare, legal advocacy, and benefits.
• Assist with applications (victim compensation, benefits, insurance).
• Coordinate care across multiple providers and systems.
• Maintain regular contact via phone, in-person, and community-based interactions.
• Advocate within healthcare, legal, and social service systems.
• Document accurately per program/funding requirements; support QA efforts.
• Participate in multidisciplinary meetings and team conferences.

SummaryThe trauma recovery center case manager provides comprehensive, trauma-informed case management services to survivors of violent crime. This role is responsible for assessing client needs, coordinating services, and supporting engagement in care across medical, mental health, legal, and social service systems.Working as part of a multidisciplinary team, the Case Manager plays a critical role in reducing barriers to care and promoting client stability, recovery, and access to resources.
