Job summary

Total Care Services, Inc. provides Supports Planning across Maryland. As a Supports Planner you help identify, access, and coordinate services and supports to maintain health, safety, and independence. You will also assist with waiver eligibility determination and maintenance.

This hybrid role is performed from a home office with local client visits, and offers a 8:00am-5:00pm schedule, Monday to Friday, with benefits including Health, Dental, Vision, and more.

Qualifications

• Minimum of 21 years of age and eligible to work in the United States
• Must have Masters or Bachelors degree in Human services field
• Must be a licensed driver with own transportation and valid insurance
• Must be a U.S. citizen or legally authorized to work in the United States
• Must have computer skills and know how to use MS Word and MS Excel
• Excellent oral and written communication skills

Responsibilities

• Get to know client and advocate for services and supports
• Identify, access, and coordinate services to maintain health, safety, and independence
• Assist with waiver eligibility determination and maintenance
• Help client develop a plan of service and achieve goals
• Help client find providers and arrange delivery of services
• Monitor services to ensure client receives needed support
• Resolve issues related to services and supports

Total Care Services, Inc. provides Supports Planning services across the state of Maryland. We help our clients access Medicaid and non-Medicaid home and community-based services (HCBS). These services are administered by the Maryland Department of Health, Office of Long-Term Services and Supports. As a Supports Planner you help identify, access, and coordinate services and supports to maintain their health, safety, and independence. You will also help with waiver eligibility determination and maintenance.

Requirements

• Minimum of 21 years of age
• MUST have Masters or Bachelors degree in Human services field.
• Must be a licensed driver and have own transportation with valid insurance.
• Must be a U.S. citizen or alien who is lawfully authorized to work in the United States
• Must have computer skills and know how to perform tasks utilizing Microsoft Word, and MS Excel.
• Must have excellent oral and written communication skills.

What You Do

• Help client find providers
• Make arrangements for delivery of services and supports
• Monitor client services to make sure they are getting the support they need
• Help resolve any issues that may occur
