GT Independence is seeking a dedicated Supports Broker (Case Manager) to coordinate Medicaid Waiver services and promote participant growth and independence. The role involves training staff, building relationships with providers and families, and helping clients access community resources to achieve ISP outcomes.
Minimum requirements include a high school diploma and at least 1 year of community experience supporting people with disabilities.

The Supports Broker (Case Manager) is responsible for coordinating the daily operation of Medicaid Waiver services, and for promoting personal growth and independence of participants in accordance with state and program regulations, GT Independence Policy and Procedures, and applicable accreditation standards.
