Job summary

Breaking Ground is seeking a Case Manager, Connect to Care, to provide outreach and case management for people experiencing homelessness. You will conduct psychosocial evaluations, help access benefits, and support transitions to permanent housing.

The role requires a Bachelor’s degree or equivalent experience, at least two years in homelessness services, and comfort with harm reduction and motivational interviewing techniques. A driver’s license is preferred.

Qualifications

• Bachelor’s Degree or equivalent work experience.
• Two years of experience working with homeless adults.
• Experience with harm reduction and motivational interviewing techniques preferred.

Responsibilities

• Provide direct services to a caseload of clients and conduct psychosocial evaluations.
• Connect clients to psychiatric services and medical treatment as needed.
• Assist clients with obtaining identification, public benefits and income sources.
• Hold weekly meetings with clients to assess needs and develop service plans.
• Prepare documentation for housing applications and assist with housing process.
