Job summary

Berkshire Medical Center, part of Berkshire Health Systems, seeks a Case Manager to coordinate patient care across the hospital continuum. You will work with a multidisciplinary team to facilitate admissions, transitions, and post-hospital arrangements, ensuring appropriate utilization and patient satisfaction.

The role requires an MSW, LCSW, and 3+ years of acute care experience, with knowledge of CMS regulations and regulatory bodies. Strong communication and negotiation skills are essential.

Qualifications

• Master of Social Work required.
• LCSW license required.
• 3+ years of acute care clinical experience.
• Experience in utilization management and discharge planning preferred.
• Knowledge of CMS levels of care and DPH regulations preferred.

Responsibilities

• Manage patient care needs and coordinate services across the hospital.
• Conduct admission and discharge risk assessments.
• Plan transitions to post-hospital care and monitor utilization of resources.
• Collaborate with multidisciplinary teams to improve quality and cost outcomes.
• Maintain confidentiality and document care plans and outcomes.
