Job summary

Urban Health Plan is seeking an entry-level Specialty Case Manager to join our Social Services team in Harlem, NY. You will provide social support, perform screenings to identify needed services, and coordinate internal and external referrals and advocacy for patients in our care.

You will collaborate with department colleagues to connect patients with community resources, strengthen care coordination, and assist with a range of casework duties.

Qualifications

• Bachelor’s degree required in a related field.
• Entry-level position; no prior case management experience required beyond degree.
• Team-player and willing to go the extra mile for patients.
• Proficiency with Microsoft Office.
• Understanding of medical terminology and social work principles.
• Commitment to excellence and quality patient service.

Position Summary:

The Specialty Case Manager will be a member of our Social Services department, working in support of patients in our CitiCares Community Health Center in Harlem, NY. The Case Manager provides social assistance, general screenings to identify support services needed, provides internal/external referrals, advocacy, and social services with patients in our care.

Minimum Qualifications

• Bachelor's Degree in Social Work, Psychology, Human Services, or related field of study.
• This is an entry level case management position, no previous experience required beyond Bachelor's Degree
• Team-player and willing to go the extra mile to support patients in our care.
• Proficiency with Microsoft Office.
• Understanding of medical terminology and social work principles.
• Commitment to excellence and to providing quality service to patients.

Work Environment Type, Location, and Schedule:

Onsite, Bronx, NY, Monday to Friday 8:00 am to 5:00 pm.

Compensation

$45,560 based on 40 regular hours per week.
