Job summary

The Doe Fund Inc is seeking a high-energy Case Manager to join our team in Brooklyn, NY. You will manage a caseload of clients, coordinate access to mental health and housing services, and work closely with DHS and other agencies to support program participants.

The ideal candidate has at least 2 years in human services, strong communication skills, and a background in case management. This role is based at our Porter Avenue Facility with a comprehensive benefits package.

Qualifications

  • A High School Diploma is required; a Bachelor's degree in a human services field is preferred.
  • At least 2+ years of experience in Human Services/Case Management.
  • Experience with challenging high-service needs populations is a plus.
  • Strong interpersonal and written/verbal communication skills required.
  • Working knowledge of MS Word, Excel, PowerPoint, and Google Suite.

Responsibilities

  • Manage a caseload of clients; monitor progress toward goals in service plans and RWA components.
  • Research and coordinate access to outside referral sources for Mental Health, Substance Recovery, Benefits, Entitlements, and Primary Care.
  • Communicate with DHS and other agencies as needed.
  • Collaborate with clients to set goals and support self-development; work as a team with staff to achieve program goals.
  • Organize and facilitate caseload and other groups on various topics.
  • Maintain current, accurate documentation of trainee progress in charts and databases.
  • Provide crisis intervention to resolve issues that may arise.
  • Perform other duties as assigned.

Join The Doe Fund on our mission to break the cycles of homelessness, addiction, and recidivism. Our flagship Ready, Willing & Able (RWA) program provides paid transitional work experience, transitional housing, advanced occupational training, and comprehensive supportive services to individuals experiencing homelessness. The Doe Fund is also one of New York City’s largest nonprofit developers of permanent affordable and supportive housing, with more than 1,900 units in operation or pre-development. The Doe Fund has an annual operating budget of $80 million and a full-time staff of nearly 500, a significant portion of whom are graduates of our programs.

The Doe Fund is currently seeking a high-energy, self-motivated experienced Case Manager to join our team. The successful candidate will have experience working with formerly incarcerated, Mental Health, Homeless, and/or Housing organizations. This position will be based out of our Porter Avenue Facility.

Key Responsibilities:

  • Perform other duties as assigned.
The Ideal Candidate:
  • A High School Diploma is required. Bachelor’s degree with a major in Psychology, Sociology, Counseling, Social Work, or other Human Services concentration preferred.
  • At least 2+ years of experience in Human Services/Case Management
  • Experience working with challenging high-service needs populations preferred
  • Strong interpersonal and written/verbal communication skills required
  • Working knowledge of MS, Word, Excel, PowerPoint, and Google
