Job summary

Qualifications

  • Capable of partnering with and empowering program residents.
  • Cultural sensitivity toward diverse populations.

Responsibilities

Job Title:Case Manager – Rosati Scattered Sites (full-time)

Job Classification: Non-Exempt (hourly)

Department:Shared Services

Summary/Objective:

The Case Manager is responsible for providing comprehensive, person-centered case management services to individuals and/or households participating in a HUD-funded Continuum of Care (CoC) Permanent Supportive Housing (PSH) program. The Case Manager works collaboratively with program participants to promote housing stability, increase self-sufficiency, strengthen connections to community-based resources, and support participants in achieving individualized goals while maintaining permanent housing.

Essential Functions:

  • Establish and maintain professional, supportive, and respectful relationships with program participants.
  • Provide individualized case management using Housing First, trauma-informed, person-centered, and strengths-based approaches.
  • Complete initial and ongoing assessments to identify participant strengths, needs, barriers, risks, and goals.
  • Collaborate with participants to develop individualized service plans based upon participant-identified priorities.
  • Review and update service plans as required and when significant changes occur.
  • Conduct scheduled participant contacts and home visits consistent with program requirements and participant needs.
  • Maintain consistent engagement with participants, including appropriate outreach when participants become difficult to contact.
  • Promote participant choice, self-determination, independence, and community integration.
  • Assist participants in developing independent living and housing retention skills.
  • Maintain HMIS Data
  • Participate in the CoC general meetings

Housing Stability

  • Assist participants with understanding lease obligations, tenant responsibilities, and applicable program expectations.
  • Identify issues that may jeopardize housing stability and intervene as early as possible.
  • Work collaboratively with participants, landlords, property management, and program leadership to address housing-related concerns.
  • Assist participants with developing strategies to prevent eviction and resolve lease violations when possible.
  • Support participants in maintaining safe and sanitary living environments.
  • Coordinate reasonable supportive interventions when health, behavioral health, financial, or other circumstances threaten housing stability.
  • Assist participants with housing transitions when a move is necessary and permitted under program requirements.
  • Support participants in pursuing longer-term housing options, rental assistance, or other housing opportunities when appropriate.

Service Coordination & Community Resources

The Case Manager will connect participants to appropriate community-based services and benefits, which may include:

  • Medicaid, Medicare, SNAP, SSI, SSDI, and other public benefits.
  • Primary and specialty healthcare.
  • Mental health and behavioral health services.
  • Substance use treatment and recovery supports.
  • Employment and workforce development.

Required education and experience:

  • Associate’s degree in related human services field. May substitute degree with minimum of five years’ case management experience involving direct work with those who are homeless.
  • Interest in providing services to people experiencing substance abuse, severe and persistent mental illness, dual diagnosis, homelessness and/or HIV/AIDS.
  • Possesses a working knowledge of community resources available to low-income housing and homeless individuals.
  • Knowledge of documentation standards, HIPAA and privacy standards, crisis management and harm reduction principles.
  • Working knowledge of Internet and Microsoft Office products including Word, Excel, and Outlook.
  • Capable of partnering with and empowering program residents.
  • Cultural sensitivity toward diverse populations required.

Preferred education and experience:

Preferred Bachelor’s in Human Services, Substance Abuse, Counseling, Social Work, or other related human services field (current licensure or certification strongly preferred).

Supervisory Responsibility: This position has no supervisor responsibilities.

Work Schedule: Generally, days, nights, and weekends as needed

Work Environment: This position operates in an emergency shelter environment.

Physical and Environmental Requirements: Ability to lift and carry at least 10 lbs.

Travel: Travel is primarily local during the business day.
