This role provides compassionate, client-centered support for individuals experiencing homelessness or housing instability. As part of PATH-HBST, you will assess needs, develop service plans, access resources, and help clients achieve long-term stability.

Catholic Charities is seeking a self-motivated advocate who is compassionate, trauma-informed, and culturally aware to join our Housing Focused Behavioral Support Team (HBST) at our St. Paul Opportunity Center / Higher Ground - Minneapolis locations.
This position is on a PATH Team (under the SAMHSA grant Project for Assistance in Transition from Homelessness) and is also known as the Housing Focused Behavioral Health Support Team. PATH-HBST is onsite Emergency Service for clients of the Catholic Charities Higher Ground Saint Paul Shelter, and the Saint Paul Opportunity Center who live with serious mental illness and often live with additional severe health conditions that require immediate support for the client’s health and safety.
This position provides direct case management to adults with serious mental illness and co-occurring conditions who are experiencing homelessness or housing instability. The Case Manager works alongside clients to assess needs, develop individualized service plans, access resources, and strengthen skills for long-term stability.
They serve as both advocate and connector - helping clients navigate complex systems of care while promoting dignity, safety, and independence. The role collaborates closely with internal teams and community partners to ensure clients receive consistent, trauma-informed support.
