Job summary

Jackson Purchase Medical Center in Mayfield, KY is seeking a Registered Nurse (RN) to join a team focused on delivering high-quality patient care. This PRN role involves coordinating with the multidisciplinary team, discharge planning, and ensuring timely treatment progression across shifts.

The ideal candidate holds an active RN license and BLS certification. ACMA Accredited Case Management Certification is required or must be obtained within 2 years of hire.

Responsibilities

• Consults with nursing staff and multidisciplinary team to evaluate patient status and care appropriateness.
• Monitors patient and family satisfaction; addresses questions and complaints from patients, families, and payors.
• Leads discharge planning, including patient transfers and referrals to community resources; provides discharge education.
• Performs chart reviews to address service delivery, patient outcomes, and reimbursement.
• Communicates with providers, payors, and ancillary services to remove barriers for timely care progression.
• Documents case management activities, plans, and outcomes accurately.
• Participates in performance improvement and required continuing education.
• Supports operations by working alternate shifts/units; may be on-call.

What you'll contribute

To perform this job, an individual must perform each essential function satisfactorily with or without a reasonable accommodation.

• Consults regularly with nursing staff and the multidisciplinary team to evaluate patient status and appropriateness of care, including admission, length of stay, transfers, and discharge.
• Monitors patient and family satisfaction; responds to questions and complaints from patients, families, and payors.
• Leads and coordinates discharge planning, including patient transfers to other facilities and referrals to community resources; provides discharge education.
• Performs chart reviews to identify and address issues related to service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.
• Communicates effectively with providers, payors, and ancillary services to remove barriers and ensure timely care progression.
• Accurately documents case management activities, plans, and outcomes.
• Participates in performance improvement and required continuing education.
• Supports department operations by working alternate shifts/units as needed; may be assigned on-call.
• Performs other duties as assigned.
