Job summary

Healthcare United is seeking a Registered Nurse/Case Manager to join our home-based health care team. You will manage patient caseloads, perform assessments, and coordinate at-home services across Stratford and surrounding towns in Connecticut.

Qualifications

• Graduate of an accredited Registered Nurse program.
• Current license as a Registered Nurse in Connecticut.

Responsibilities

• Must Case Manage 10 patients and complete at least 5 assessments per month.
