Qualifications

• Associate degree in nursing or nursing diploma required.
• Bachelor’s degree in nursing preferred.
• 2+ years in case management or 3+ years in clinical nursing required.
• Currently licensed as an RN in practice state(s).
• InterQual experience preferred.
• Case management certification preferred.
• Strong communication and interpersonal skills.
• Analytical and proactive decision-making abilities.

Responsabilités

• Coordinate services and develop care plans for patients and families.
• Guide patients and families through program orientation and discharge criteria.
• Collaborate with physicians, nurses, and leaders to ensure safe transitions.
• Document psychosocial assessments and communicate needs clearly to the care team.
