Job summary

Wilcox Medical Center is seeking an RN Case Manager to ensure confidential management of high-risk patient cases, performing case management through referrals and medical record reviews. The role emphasizes discharge planning, treatment plan assessments, and coordination with multiple services to ensure quality care.

Location: Wilcox Medical Center. Work Schedule: Day - 8 Hours. Full Time Regular, FTE 1.0. EOE/AA/Disabled/Vets. Pay range listed as $127,691–$159,619 per year.

Qualifications

  • Bachelor's Degree and/or equivalent combination of education, training, and/or experience.
  • Current Hawaii RN License.
  • Three (3) years of clinical experience in acute care, community or long term care, or related field.

Responsibilities

  • Perform case management through direct referrals and reviews of medical records.
  • Determine the appropriateness and medical necessity of admissions, continued hospital stay, and use of ancillary services.
  • Perform treatment plan assessments, discharge needs interviews, planning/coordination for high risk cases, and participate in organizational quality improvement activities.

Founded in 1938, Wilcox Medical Center is a not-for-profit medical center dedicated to providing the Kaua‘i community with accessible quality health care. The largest medical center on Kaua‘i, it is a state-of-the-art acute care facility with a full suite of services offering 30 specialties and programs, including cardiology, emergency, family practice, gastroenterology, health management, internal medicine, neurology, OB-GYN, oncology, orthopedics, pediatrics and urology. Its 18-bed emergency department serves as the island’s Primary Stroke Center. The medical center also has four birthing suites, seven intensive care beds and 20 same-day surgery beds. Wilcox is the first American College of Surgeons-verified Level III Trauma Center in the state of Hawai‘i. Wilcox is part of Hawai‘i Pacific Health, one of the state’s leading health care systems and a not-for-profit health care organization with medical centers, clinics, physicians and other caregivers working together to create a healthier Hawai‘i.

The Case Management team puts great emphasis on helping each patient transit their health care treatment with an appropriate set of support tools and information. Our caring, professional staff provides timely medical and discharge management to meet clinical and service goals, and to provide an effective, appropriate continuum of care. We want to ensure that our patients receive the attention they need, especially when they require care from several different services and other combinations of medical, financial or social intervention.

As the RN Case Manager, you will ensure the considerate and confidential management of unique and high-risk patient cases by performing case management through direct referrals and reviews of medical records. This includes determining the appropriateness and medical necessity of admissions, continued hospital stay, and the use of ancillary services. You will also perform treatment plan assessments, discharge needs interviews, planning/coordination for high risk cases, and participate in organizational quality improvement activities. We are looking for someone perceptive to patient needs, with outstanding organizational and communication skills and a commitment to delivering the highest quality health care to Hawai‘i’s people.
