Job summary

The Judge Group seeks a Case Manager RN in Bangor, ME for a 4+ month assignment. The role encompasses admission and ongoing case management, discharge planning, and coordination across disciplines to optimize care progression and ensure CMS compliance.

Responsibilities include securing reviews, coordinating post-acute placements, maintaining clinical documentation, and supporting audit readiness. Shift runs 08:00–16:30 with a varied schedule including weekends.

Qualifications

• Bachelor's degree and active RN license required.
• Case management experience is essential.
• Direct patient-facing or bedside care experience.

Responsibilities

• Conduct admission and ongoing case management assessments for medical necessity and level of care.
• Drive discharge planning to optimize length of stay and prevent delays.
• Ensure CMS compliant reviews and secure documentation for admissions and continued stay.
• Coordinate transitions and post-acute placements to avoid readmissions.
• Maintain timely clinical documentation supporting medical necessity and regulatory requirements.
• Partner with physicians, nursing, social work and ancillary teams to resolve discharge barriers.
• Support audit readiness and CMS participation.
• Monitor high-risk populations and identify early interventions.
• Adhere to organizational policies and CMS Conditions of Participation.

Job Title: Case Manager RN

Location: Bangor, ME 04401

Duration: 4+ Months (possibilities of extension)

Shift: 08:00 AM - 04:30 PM (Varied schedule including weekdays and weekends)

Job Responsibilities

• Conduct comprehensive admission and ongoing case management assessments, including medical necessity, level of care validation, and progression of care.
• Drive length of stay optimization and access day performance through proactive discharge planning and daily interdisciplinary collaboration.
• Ensure CMS compliant reviews and form securement, including timely admission reviews, continued stay determinations, IMM and MOON administration.
• Coordinate care transitions, post-acute placement, to prevent avoidable delays and readmissions.
• Maintain accurate, timely clinical documentation supporting medical necessity, care progression, and regulatory requirements.
• Partner with physicians, nursing, social work, and ancillary teams to resolve barriers to discharge and align care plans.
• Support audit readiness and compliance.
• Monitor high-risk populations and identify opportunities for early intervention to reduce excess days.
• Adhere to organizational policies, standards, and CMS Conditions of Participation.

Required Qualifications

• Bachelor's Degree.
• Active RN License.
• Case Management experience.
• Patient-facing/bedside presence experience.
