Marshall Medical Center is seeking a Case Manager RN to coordinate patient care and resources, promote wellness, and ensure cost-effective outcomes across the continuum of care. The role emphasizes collaboration with the health team and community resources in California.

The Case Manager is a Registered Nurse (RN) who uses a collaborative process which assesses plans, implements, coordinates, monitors and evaluates options and services to meet an individual’s health needs through communication and available resources to promote quality cost-effective outcomes. The Case Manager applies each of these six essential activities within each of the following five core components: Coordination and Service Delivery, Physical and Psychological Factors, Benefit Systems and Cost Benefit Analysis, Case Management Concepts, and Community Resources. These activities and core components are applied to promote wellness on the continuum of care.

Marshall’s executive leadership is governed by a local board of directors who volunteer their time to represent the needs and interests of a growing community.
