Santa Clara Post Acute in Tacoma, WA is seeking a Full-Time Case Manager (RN) to lead care coordination for residents across the care continuum. The successful candidate will hold a current RN license, demonstrate strong clinical judgment, and work independently to advocate for residents, educating families as needed.

Qualifications

• Graduated from an approved and certified RN program.

• Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines for long-term care.

Your Key Responsibilities Include

• Managing care for residents throughout the care continuum, including pre-admission, admission, and post-admission stages.
• Coordinating services to ensure that residents' needs are met in a timely and effective manner.

• Advocating for residents and families, ensuring their needs are met, and providing education as needed.

Must Have

• Successfully graduated from an approved and certified RN program
• Current and unencumbered license to practice as an RN in the state
• Ability to make quick judgments and work independently
• Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care

Nice To Have

• 2 years of experience as a RN
• Active patient or bedside care experience within the past three years
