New Vista Of The Bluegrass Inc in Louisville, KY is seeking a Case Manager-RN for a full-time position at Bingham Gardens. You will manage caseloads, attend appointments, accompany clients to medical visits, and coordinate care between care teams to support residents with developmental disabilities.

The role requires an Associate or Bachelor degree in Nursing and an RN license (Compact State). Salary ranges from $42.47 to $45.89 per hour, with a sign-on bonus and comprehensive benefits including

Qualifications

• Must possess an Associate or Bachelor degree in Nursing and a valid Registered Nurse license issued by the Kentucky Board of Nursing or a state Nursing Board in the “Compact State Agreement”.

Responsibilities

• Attends all client related meetings on their assigned case load.
• Serves as the liaison between client care teams.
• Attends medical appointments with clients as needed.
• Make rounds on assigned homes and to hospitals as needed when clients are hospitalized; providing written update to the team.
• Complete all monthly physical assessments on clients.
• Completes all nursing assessments.
• Attend specialty clinics as needed or requested.
• Ensure immunization records are up-to-date.
• Responsible for writing, monitoring and updating SAMS learning objectives and recording into Key Indicators
• Write mortality reviews
• Completes pre-admission assessments for potential clients, all needed paperwork for new admissions as needed.
• Assist with transition training related to client care
• Serves on facility committees, attends in-service training programs, participates in drills, and responds to facility emergencies.

Required Education and Experience

Position Type/Expected Hours of Work

Typical hours for this position are M-F 8AM - 4:30p. This is aFULL TIME 37.5 hour work week position. Employee must be able to flex hours/work overtime as needed to meet facility needs. Bingham Gardens is a community home setting serving clients with intellectual and developmental disabilities.

Essential Functions

• Attends all client related meetings on their assigned case load.

• Write mortality reviews
• Completes pre-admission assessments for potential clients, all needed paperwork for new admissions as needed.
• Assist with transition training related to client care

Supervisory Responsibility

This position requires supervision of LPNs and RNs

Travel

Travel may be expected to cover nursing staff at the Group Homes location.
