Job summary

Sevita seeks a Case Manager to join NeuroRestorative in a full-time role, offering a salary range of $45,000 to $65,000. The case manager will assess needs, plan care, and coordinate services to support independent living for individuals with brain, spinal cord, or medically complex injuries.

Ideal candidates hold a bachelor’s degree in human services, have at least 1 year of related experience, and possess a valid driver’s license.

Qualifications

• Bachelor’s degree in human services or equivalent in education and experience.
• Minimum of 1 year of related work experience and knowledge of case management.
• Valid driver’s license and insurability as defined by the company.
• Current CPR/First Aid Certification as required by state/program.
• Knowledge of case management and ability to multitask.
• Strong communication skills, ability to present to groups and respond to questions.
• Ability to analyze data, write reports, and use metrics and Excel.

Responsibilities

• Provide case management services including intake, assessment, crisis intervention, advocacy, referral, and monitoring.
• Gather social history to assess needs and family strengths.
• Conduct home visits and accompany individuals as needed.
• Coordinate care with outreach workers and other professionals to avoid duplication.
• Develop and monitor plans of care including advocacy, independent living skills, and therapies.
• Assist with benefits eligibility and maintain case records with progress notes.
