Job summary

Sevita is seeking a Case Manager to provide community-based services for individuals with intellectual and developmental disabilities. Responsibilities include case management services such as intake, assessment, and crisis intervention while ensuring individuals receive high-quality care.

Qualifications

• One year of related work experience in case management.
• Current CPR/First Aid Certification as required by state/program.
• Valid driver’s license with insurance.

Responsibilities

• Provide case management services including intake, assessment, and monitoring.
• Conduct home visits and accompany individuals being served.
• Develop a comprehensive plan of care for each individual.
