Position Summary

Schedule: Monday through Friday 8:00 AM - 5:00 PM EST

Travel: Up to 55-75% within designated area

Join our Community Care team to elevate patient care! Facilitate the delivery of benefits and healthcare information, develop health strategies, and support member wellness and return to work. Utilize a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy to meet health needs and promote quality, cost-effective outcomes.

Responsibilities

• Act as liaison with members, families, providers, insurance companies, and healthcare personnel.
• Coordinate case management activities for catastrophic and chronically ill members, including referrals, home visits, and resource utilization.
• Assess and analyze medical and vocational status; develop care plans to optimize health and benefits eligibility.
• Communicate with stakeholders via phone or in person; document all case activities.
• Consult with multidisciplinary teams; make outreach to physicians and specialists as needed.
• Provide educational and preventive information; comply with relevant laws and regulations.
• Evaluate needs and benefits, facilitate care, and monitor progress toward outcomes.

Qualifications

• Active Pennsylvania RN license
• At least 3 years of clinical experience, preferably with Medicare members (e.g., diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac)
• Minimum 2 years in case management, discharge planning, or home health care
• Willingness to travel 55-75% within Franklin or Adams Counties
• Preferred: Certified Case Manager, additional professional certifications (CRC, CDMS, CRRN, COHN, CCM), Bachelor's or Nursing Diploma

Compensation and Benefits
