Case Manager, Registered Nurse (Pennsylvania)

Position Summary:

Schedule: Monday through Friday 8:00 AM - 5:00 PM EST.
Travel: Up to 55-75% within Franklin or Adams Counties.

Join our Community Care team to enhance patient care through a member-centric, community-based care management model. Responsibilities include assessing, planning, coordinating, and advocating for members' health needs, collaborating with providers and community organizations, and promoting wellness activities.

Key Responsibilities:

• Coordinate case management activities for catastrophic and chronically ill members.
• Interact with members in person or telephonically, including home visits.
• Assess medical and vocational status, develop care plans, and facilitate appropriate care and benefits.
• Communicate with stakeholders and document case activities.
• Consult with multidisciplinary teams and healthcare providers.
• Provide education and prevention information.
• Ensure compliance with laws and regulations, and testify as needed.
• Monitor progress and evaluate outcomes.

Qualifications:

• Active Pennsylvania RN license.
• 3+ years clinical experience (preferred in diabetes, CHF, CKD, post-acute, hospice, cardiac).
• 2+ years in case management, discharge planning, or home health.
• Willingness to travel 55-75%.

Preferred Qualifications:

• Excellent analytical, communication, and organizational skills.
• Ability to work independently, possibly remotely.
• Proficiency in MS Office and proprietary applications.
• Certified Case Manager or other relevant certifications preferred.

Compensation: $60,522 - $129,615 annually, plus benefits and incentives.
