Johns Hopkins Care at Home in Baltimore, MD is seeking a Home Care Pediatric Discharge Planner to coordinate services for pediatric patients transitioning from hospital to home. This role acts as a discharge planning resource and does not provide hands-on clinical care. Work Location: JHH.

Schedule: Days, 40 hours per week. Qualifications include a BSN, RN licensure, active CPR, and at least two years of clinical nursing experience; prior home care or discharge planning is preferred.

The Home Care Coordinator Pediatric Discharge Planner specializes in coordinating home care services and resources for pediatric patients transitioning from hospital to home, acting as a discharge planning resource without providing hands-on clinical care.

Work Location: JHH

Weekly Working Hours: Days, 40 hours per week

Qualifications

• Bachelor of Science in Nursing (BSN) required
• Licensure in clinical discipline (RN)
• Active CPR
• Two (2) years experience in clinical nursing required , previous home care or home care discharge planning experience preferred
• Knowledge of Medicare Conditions of Participation preferred
• Thorough knowledge of home care community resources, agency policy and procedures, federal and state regulations and third party regulations for reimbursement of home care services preferred.

Salary Range: Minimum 43.61/hour - Maximum 67.59/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
