Caris Healthcare is seeking a Part-time or PRN Case Manager Registered Nurse in Johnson City, TN. The RN will assess, plan, implement, and evaluate total patient care under physician orders for hospice patients, coordinating care and education for patients and families.

The role requires RN licensure in the operating state, hospice or home health experience preferred, and offers a benefits package with flexible scheduling and opportunities for growth.

Qualifikationen

• Must be a Registered Nurse licensed in the state of operation.
• Nursing experience required, hospice or home health preferred.
• Must possess a sound knowledge of nursing principles, procedures and elements of patient/family teaching.

• Collaborate with patient, family, partners, and interdisciplinary team to develop individualized plans of care for patients.

• Observe and evaluate patient’s physical and emotional status, safety factors, and family dynamics to determine education needs for caregivers.

Position: Registered Nurse, RN

Pay: $70,000 - $73,000 yearly Depending on Experience

Caris HealthcareRN Case Manager provides nursing care to patients per the physician’s orders.
