Position Summary

This position offers a $1,000 Sign on Bonus for external applicants! The Case Manager Registered Nurse position is with Aetna's Integrated Care Management (ICM) team and is a field-based role serving the Southern and Lower Central Region of New Jersey, preferably Ocean, Monmouth, or Atlantic County. Candidates must hold a NJ Registered Nurse License and be willing to travel 50-75% to meet with members face-to-face.

The care manager assesses members' needs through telephonic and face-to-face evaluations in various settings, including members' homes, hospitals, behavioral health, and long-term care facilities. They develop and monitor care plans collaboratively with members, caregivers, and providers, ensuring safe discharges and addressing social needs like housing and food security. Accurate documentation and autonomous work are essential, and mentorship of new hires may be required.

Required Qualifications

• Must reside in Southern and Lower Central Region of New Jersey; preferably Ocean, Monmouth, or Atlantic County.
• 3+ years' experience with Pediatric, Special Needs, and/or Adult populations.
• 2+ years managing populations with mental health and/or substance use disorders.
• RN with current unrestricted NJ license.
• Reliable transportation and ability to travel 50-75% of the time; mileage reimbursed.
• Proficiency in computer systems and MS Office Suite.

Preferred Qualifications

• Certified Case Manager Certification.
• Experience in an integrated case management model.
• Bilingual skills preferred.
• Effective verbal and written communication.
• Knowledge of medical terminology and digital literacy.
• Ability to handle sensitive information ethically.

Education

• Bachelor's degree in Science & Nursing required.

Additional Details
