Case Manager Registered Nurse - Field Must reside in Southern and Lower Central Region of New Jersey - $1,000 Sign on Bonus

Position Summary: This position offers a $1,000 Sign on Bonus for external applicants! It is with Aetna's Integrated Care Management (ICM) team and is a field-based role out of the Southern and Lower Central Region of New Jersey, preferably Ocean, Monmouth, or Atlantic County. Candidates must hold a NJ Registered Nurse License and be willing to travel 50-75% of the time to meet members face-to-face.

The care manager assesses and evaluates members with potential care management needs through telephonic and face-to-face assessments in various settings, including the member's residence, hospitals, behavioral, and long-term nursing facilities. They establish and monitor care plans, coordinate services, and advocate for members' needs, including social needs like housing and food security. Accurate documentation in electronic health records is essential. The role requires critical thinking, problem-solving, and autonomous working skills, with potential mentorship responsibilities.

Required Qualifications:
  • Reside in Southern and Lower Central Region of New Jersey, preferably Ocean, Monmouth, or Atlantic County
  • 3+ years' experience in Pediatric, Special Needs, and/or Adult populations
  • 2+ years managing populations with mental health and/or substance use disorders
  • RN with current, unrestricted NJ licensure
  • Reliable transportation and willingness to travel 50-75% with reimbursed mileage
  • Proficiency in computer systems and MS Office Suite
Preferred Qualifications:
  • Certified Case Manager Certification
  • Experience in an integrated case management model
  • Bilingual skills
  • Strong communication, problem-solving, and medical terminology knowledge
  • Digital literacy skills and ability to handle sensitive information ethically
Education:
  • Bachelor's in Science & Nursing required
Additional Details:

Full-time, 40 hours/week, with a pay range of $72,627 - $155,538, depending on experience and location. Benefits include medical, 401(k), stock purchase plans, wellness programs, paid time off, flexible schedules, and more. Application window closes on 07/10/2025.
