Case Manager Registered Nurse - Field Must reside in Southern and Lower Central Region of New Jersey - $1,000 Sign on Bonus

Join us for the Case Manager Registered Nurse - Field Must reside in Southern and Lower Central Region of New Jersey - $1,000 Sign on Bonus role at The Muse.

Position Summary

This position offers a $1,000 Sign on Bonus for external applicants! It is with Aetna's Integrated Care Management (ICM) team and is a field-based role based in the Southern and Lower Central Region of New Jersey, preferably Ocean, Monmouth, or Atlantic County. Candidates must hold a NJ Registered Nurse License and be willing to travel 50-75% of the time to meet members face-to-face.

The care manager assesses and evaluates members with potential care needs through telephonic and face-to-face assessments in various settings, including private residences, hospitals, behavioral, and long-term nursing facilities. The role involves developing member-centric care plans, monitoring their effectiveness, and adjusting them as needed, collaborating with members, caregivers, and providers. The care manager advocates for the member's needs, ensuring safe discharges and addressing social needs, with accurate documentation in electronic health records. Critical thinking, problem-solving, and autonomous work are essential, and mentorship of new hires may be required.

Required Qualifications
  • Must reside in Southern and Lower Central Region of New Jersey, preferably Ocean, Monmouth, or Atlantic County
  • 3+ years' experience in Pediatric, Special Needs, and/or Adult populations
  • 2+ years managing mental health and/or substance use disorders
  • RN with current, unrestricted NJ license
  • Reliable transportation and willingness to travel 50-75%; mileage reimbursed
  • Proficiency in computer systems and MS Office Suite
Preferred Qualifications
  • Certified Case Manager Certification
  • Experience in an integrated case management model
  • Bilingual abilities
  • Effective communication skills
  • Knowledge of medical terminology and digital literacy
  • Ability to handle sensitive information ethically
Education
  • Bachelor's degree in Science & Nursing
Additional Details

Full-time, 40 hours/week. Pay range: $72,627 - $155,538, based on experience and location. Eligible for bonuses and benefits including health plans, 401(k), stock purchase, wellness programs, paid time off, flexible schedules, and more. Application deadline: 07/10/2025.
