Qualifications

• Prior experience in homecare or hospice sales/business development.
• Background in case management and/or medical social work.
• Strong relationship-building and networking skills.
• Solid organizational and case management abilities.

Responsibilities

• Serve as the primary case manager for an assigned caseload — assessing needs and creating care and discharge plans.
• Build relationships with hospital discharge planning teams, physicians, and referral sources to keep referrals flowing.
• Provide daily updates on pending referrals and follow up on new ones.
• Grow SCN's referral network using relationship-building and sales instincts.
• Coordinate care conferences among physicians, providers, and family to build feasible plans.
• Prepare discharge summaries to insurance providers to secure coverage.
• Maintain accurate case documentation compliant with regulatory requirements.
