Homestead Healthcare in Michigan seeks a Community Liaison to lead referral generation and drive business growth for our home care services. This is a results-driven role requiring independence, relationship-expertise, and ownership of referral metrics that impact profitability.

You will build partnerships with hospitals, SNFs, hospice, and clinics, maintain a weekly presence, and collaborate with intake and care teams to onboard clients.

Qualifications

• 3+ years in community outreach, marketing, or liaison roles in healthcare.
• Proven track record of meeting referral quotas.
• Excellent communication and presentation skills.
• Strong network of referral sources in the local counties.

Responsibilities

• Develop and execute a referral growth strategy with hospitals, SNFs, and physicians.
• Leverage relationships to generate qualified home care referrals.
• Maintain 10-15 hours weekly presence at referral partners.
• Meet or exceed monthly and quarterly referral targets.
• Track referral activity and pipeline using CRM.
• Provide market feedback to inform strategy.
• Represent the company at community events and health fairs.
• Collaborate with intake and care teams to onboard referred clients.
• Flexibility to assist in other capacities as needed.
