Job summary

Boca Recovery Center is seeking a Case Manager to support clients through treatment by coordinating case management, discharge planning, and advocacy. The role requires strong organization, empathy, and the ability to link clients with medical, financial, and social resources.

Responsibilities include assessments, scheduling, and collaboration with a multidisciplinary team to ensure seamless care and successful aftercare planning for lasting recovery.

Qualifications

• Valid State Driver's License required.
• CPR certification required or obtained within 30 days of hire.
• 1 year of experience in substance abuse treatment or related field preferred.
• Understanding of addiction behavior and recovery support services.

Responsibilities

• Case Management & Client Support: Complete initial and ongoing case management assessments; evaluate client needs across medical, psychological, social, financial, and employment domains; assist with scheduling medical, dental, and other appointments.
• Discharge & Aftercare Planning: Coordinate discharge planning, including travel, and scheduling outpatient or counseling services; arrange placement in halfway houses or aftercare facilities as needed; ensure continuity of care.
• Documentation & Advocacy: Maintain accurate client progress documentation; uphold confidentiality and standards; advocate for client needs respectfully.
• General Responsibilities: Maintain prompt attendance; work with interdisciplinary team; uphold policies and ethical guidelines; perform other duties as assigned by Director of Operations.
